You do, or rather your attorney and his/her staff does. The district where you file may have a prescribed format for a Chapter 13 plan, which you of course must follow. The exact way in which your plan classifies debts and proposes to repay them is, however, mostly up to you and is central to your case. The trustee will review the official documents, may require modifications or amendments before approving the finished product. If you later discover inadvertent errors, notify the trustee or your attorney immediately. Some may be significant, such as omitting an asset, and require more paperwork.
